New Patients
New patients must complete the new patient intake form before the first appointment. The new patient intake form will be sent to the email registered after making the appointment on Massagebook. Patients may also fill out a physical copy in the office but should arrive 15 minutes prior to their scheduled appointment.
New patients should schedule a ‘New Patient’ time slot and Returning patients should schedule a ‘Follow Up’ Time Slot.
